PLEASE NOTE! To work reliably, fillable PDF documents need to be downloaded and saved on your computer. After downloading, use the free Adobe Reader software to open and complete the form.
If you don't have Adobe Reader on your computer, it is available here (you can deselect any "Optional Offers" available on the Adobe Reader download page).
Following are two options for downloading fillable PDF documents to your computer from commonly used web browsers:
OPTION 1
- Position your cursor on the PDF document link and click with your right mouse button.
- From the menu that pops up, select the Save target as... option in Internet Explorer or the Save link as... or similar option in other browsers.
- You should then be prompted to choose a location where the PDF document will be saved.
- Select your preferred location and click on the Save button.
- Navigate to the location where the PDF document was saved and open the document with Adobe Reader.
OPTION 2
- Position your cursor on the PDF document link and click with your left mouse button to open the PDF document in your web browser.
- After the PDF document is open in your web browser, then following are directions for various browsers to save the document to your computer or network. Be sure to note where you are saving the PDF document on your computer so you can find it and open it with Adobe Reader.
- Navigate to the location where the PDF document was saved and open the document with Adobe Reader.
- In Chrome you may either get a popup box shown below or you may need to hover over the top of the page and you will get a download arrow as shown below.

In Firefox, you can go to "File" on menu bar and select "Save Page As" as shown below or there may be a download icon in the upper right hand of the page (see below).

- In Microsoft Internet Explorer you may be able to complete the form in your browser but it is recommended that you go "File" on the menu bar, select "Save As" or use the icon on the upper left hand of the page to save the document to your computer (see below) .

- In Microsoft Edge you may need to display a toolbar in order to download the PDF document. The tool bar can be displayed by right clicking anywhere on the document and selecting "show toolbar." After the toolbar is displayed, use the icon shown below to download the document.

If you are using some other type of web browser, please use whatever means it provides to save the PDF to your computer and then open the PDF with Adobe Reader.
