Forms and Procedures required for Obtaining a Permit to Drill, Deepen, or Re-enter an Oil & Gas Well
The following forms/applications are available for either downloading or completion online, or both:
- Downloadable forms are available in a **PDF format and may be printed out and completed by hand.
- Online forms are available in a PDF format to allow you to complete the form online and then print and mail the form.

Included with each form is a link to the pertinent section of South Dakota's Oil and Gas Conservation Rules, Administrative Rules of South Dakota (ARSD) Article 74:12 section that corresponds to that form.
